Remember to sign the bottom of the second page (as required by the the Archdiocese of Vancouver)

You can drop off the completed form at the parish office or in the Sunday collection.

Pre-Authorized Payments (PAP)

If you are NEW to the program, please complete the Form and return it with a void cheque to the office. Please do not forget to sign the back as it is invalid without a signature. You may seal the completed form in an envelope marked Pre-Authorized Payment and drop it in the Collection Basket, mail it or bring it in person. The form must be in the office by the 20th of the month in order for it to be effective on the 1st of the month following. Please indicate at the top of the form that this is a new payment plan.
